We are seeking individuals to join our team and work with NDIS participants in the Regional New South Wales and Regional Victoria.
About the Role
Attend Care is looking to recruit a NDIS Support Coordinator/Recovery Coach to assist participants who require services under their NDIS plan. To enhance the lives of participants, assisting them to achieve their goals and live their best lives.
As a Support Coordinator, you will be responsible for assisting National Disability Insurance Scheme (NDIS) participants in activating, interpreting and implementing their NDIS Plans, with a strong focus on seeing participants move towards their NDIS goals. You will work closely with NDIS participants and their families/guardians to connect with individual services and supports outlined in their NDIS Plan, ensuring they get most out of their funding and achieve desired outcomes.
In this role, you will support the client with daily living activities, personal care, community participation and achieving their individual goals. You will be part of a collaborative and experienced team, working together to provide consistent, respectful and high-quality support.

A variety of shifts are available, including mornings, evenings and weekends.
You will be someone who is reliable, compassionate and able to work well within a dedicated team of experienced and passionate support professionals.
Requirements
- NSW Drivers Licence or working towards and ability to obtain
- NDIS Quality and Safeguarding Commission Training Module ‘Quality Safety and You’

What will my Job involve and Responsibilities include:
- Work in consultation with participants and their families to identify services and supports that align with their needs and available funding.
- Research, coordinate, and manage a range of supports to suit individual needs.
- Respond to new support co-ordination enquiries for new participants.
- Meeting their goals and exercising choice in obtaining supports that assist them in pursuing their own lifestyles.
- Provide support to participants to increase their independence, social participation, and economic participation so they can take more control of their lives and to better manage the complex challenges of day-to-day living
- Assist participants, families and carers to connect with services both mainstream and within disabilities
- Develop, monitor, review and report on your participant’s NDIS plans and share progress with key stakeholders.
- Work within the funding allocated and regularly monitor and track billable coordination hours.
- Building and maintaining relationships with our clients, their families, staff, peers, other services and stakeholders.
- Identification of risks to our customers using person centred practices and how to implement both short term and long-term management strategies.
- Providing support to individuals and their families.
- Promoting an environment that encourages our customers to feel safe and comfortable.
- Identifying individual’s needs and following and contributing to their support plans.
- Providing practical support for individuals and their families, such as assisting with household tasks, personal care (showering) and meeting goals in their individual Plans.
- Supporting and monitoring their healthcare needs, including administering medication eating and drinking and other individual health requirements.
- Supporting persons with disability to pursue hobbies, interests and learning new skills
- Teaching life skills, such as budgeting and paying bills.
- Helping with everyday tasks such as Doctors Appointments, meal preparation, writing a shopping list, going shopping, or maintaining their housing tenancy.
- Helping people to access community facilities and participate in Community activities according to their individual goals.
- Understanding their communication needs and adapting your communication to each individual, e.g. verbal, sounds, tones, body language.
- Working with other professionals, such as doctors and therapists, to ensure consistency of support to enhance their daily lives.
- Deliver person-centered support to Participants in a Supported Independent Living setting
- Deliver person-centered support to Participants in a Short-Term Respite setting
- Support Participants in their homes with daily tasks, personal care, and medication administration
- Support Participants to participate in their local communities and engage in meaningful recreational activities
- Build enduring and positive relationships with our Participants
- Support Participants to Lead their Lives!
The Psychosocial Recovery Coach role:
- Develops recovery-enabled relationships, based on hope
- Supports NDIS participants with recovery planning
- Coaches NDIS participants to increase recovery skills and personal capacity, including motivation, strengths, resilience and decision-making
- Supports NDIS participants to engage with the NDIS
Qualifications
- Certificate 111 in Individual Support (Ageing & Disability) or the ability and willingness to obtain
- Diploma or BSc in Nursing as a base qualification. Qualifications in Social Work, Allied Health, Case Management or equivalent experience is desirable. Relevant tertiary qualifications in psychology, mental health, health or welfare degree, or Certification in mental health and substantial years of relevant experience, or equivalent level of expertise attained through previous employment, service, and study.
- Australia Mental Health First Aid or the ability and willingness to obtain
- Current Australian First Aid Certificate & CPR, or the ability and willingness to obtain
- Covid 19 training or ability and willingness to obtain
- Medication Training (or willingness to obtain within 3 months)
- Manual Handling Training (or willingness to obtain within 3 months)
- Demonstrated capacity to support participants who may engage in behaviours of concern, utilising a positive and proactive approach.
- Candidates who have experience implementing complex Behaviour Support Plans are encouraged to apply.
- Sound IT competence with Google workspace, Consumer Management Systems, electronic timesheets, and ability to adapt to new systems as they are introduced. Highly computer literate
